Job Description

Job Summary: Provides case management and counseling services to Holocaust Survivors. 

Job Responsibilities 

  1. Assesses client’s emotional, social, environmental and financial status. 
  2. Develops care plan with client and relevant collaterals (e.g., family members, physicians, etc.).
  3. Provides accurate and appropriate information and referral assistance.
  4. Arranges for provision of services (e.g., home care, meals-on-wheels, etc.). 
  5. Assists clients with application for benefits/entitlements.
  6. Advocates on behalf of clients with other agencies. 
  7. Provides casework/case management services to clients, monitors cases and has on-going contact with clients and other collaterals.
  8. Provides counseling services to clients, on an on-going basis and/or during crisis situations. 
  9. Organizes, facilitates, and assists at social, recreational and other group activities and events for clients.
  10. Represents the organization at community meetings, committees, and inter-agency councils. 
  11. Performs client intake duties by phone or walk-in. Takes information from client or collateral to determine eligibility and need.
  12. Becomes knowledgeable about entitlement programs and benefits for our specific client population. 
  13. Maintains case notes and records, and required documentation, according to departmental standards and/or regulatory requirements. 
  14. Prepares statistical and other reports, as required. 
  15. Performs other related duties as assigned or required.

Knowledge, Skills And Requirements
  •  Master’s degree in Social Work (M.S.W.) or related field. 
  •  Experience in geriatrics preferred. 
  •  NYS license and registration to practice as a Licensed or Certified Social Worker in New York State preferred.
